Responsibilities
- Coordinate patient care plans across multidisciplinary teams
- Manage electronic health records and documentation systems
- Facilitate communication between patients, families, and healthcare providers
- Ensure compliance with HIPAA and healthcare regulations
- Assist with patient admissions, discharges, and transitions
- Monitor patient progress and adjust care plans as needed
- Collaborate with insurance companies for authorization processes
Qualifications
- Associate's degree in healthcare or related field (Bachelor's preferred)
- 1+ years of healthcare coordination or clinical experience
- Certified Medical Assistant (CMA) or similar certification required
- Proficiency with EHR systems (Epic experience highly valued)
- Strong interpersonal and communication skills
- Ability to work in high-pressure, fast-paced environment
- Valid Wisconsin driver's license and reliable transportation
